A Plague Tale: Requiem is the long-awaited next step in Amicia and Hugo de Rune’s journey, which sees them facing deadly swarms of rats yet again while seeking a cure for the boy’s illness. Although it’s a mostly linear game, its new game+ mode acts as one incentive to replay the story once you’ve seen it through once.
As its name suggests, A Plague Tale: Requiem’s new game plus mode isn’t available right from the start. For your first run of the game, you can choose between three difficulties and freely swap between them as you play.
A Plague Tale: Requiem New Game+ Mode
To unlock A Plague Tale: Requiem’s new game+ mode, you’ll actually have to beat its story once. This includes wrapping up all of its chapters until you get a screen informing you that you can now access the mode alongside a new skin for Amicia’s crossbow.

From that point on, you can convert that save to a new game+ save. Doing so carries over the codex entries, upgrades, and skills from your first playthrough while resetting your progress.
This means that you won’t have access to your previously unlocked chapter list until you beat them again.
The mode also locks your game to the new Ultimate difficulty which, from what I can tell, cannot be changed in the settings menu.
It’s a decision you cannot revert, so it’s a good idea to be sure you want to give it a shot before converting your save game.
If you’re looking for an extra reason to replay A Plague Tale: Requiem after completing it, the new game+ mode should provide a more challenging experience while also giving you the chance to find any remaining codex entries as well as unlock any skills and upgrades you might have missed the first time around.
