Developer CD Projekt Red has confirmed that The Witcher 3’s next-gen update will launch with cross-save support.
This essentially means that you will be able to transfer your The Witcher 3 saves when moving between different platforms.

The whole process relies on having an active GoG account. According to a support article on the game’s website, you’ll also want to make sure that you have access to the same in-game content on the platform to which you’re transferring your saves.
On PC, you can check whether or not the feature is turned on by heading into the Load Game menu and pressing C.
If The Witcher 3’s cross-save support is enabled, your most recent autosave, checkpoint, and manual save are uploaded to the cloud, which makes them available for transfer to other platforms.
